Family Dentistry in Maple

Our office is conveniently located at the northeast corner of Keele St. and Rutherford Rd. close to the Rutherford Rd. GO Station. We have been a proud member of the Maple/ Vaughan community for over 18 years.

Our goal is to make all of our patients part of a growing dental family where they feel comfortable. We provide a wide range of general dental services for children and adults with a focus on dental implants, smile reconstruction, cosmetic dentistry and the treatment of gum disease.

We also provide orthodontic services to our patients using Invisalign. Patients can also choose to have sedation during treatment to provide the most relaxing experience possible. Our patients experience a very positive and enthusiastic environment that will exceed their expectations.

Dr. Michael Forman - Vaughan Dentist


Dr. Forman graduated with distinction in 1996 from The University of Western Ontario. He and his wife, Fara, have two daughters (Jordyn, 18 yrs. and Samantha, 16 yrs. and a son (Joshua, 13 yrs.). Dr. Forman and his family live a short drive from the office. Dr. Forman started practicing family dentistry in Maple in 1998 and has been happily growing his dental family of loyal patients.

We have been in our present location for 18 years. Dr. Forman loves dentistry and always enjoys meeting new patients. Although we practice general dentistry for children and adults, Dr. Forman is truly inspired by the advances in dentistry that allow him to help patients look and feel great. Dr. Forman’s focus is on implant dentistry, cosmetic and reconstructive dentistry and in treating gum disease, along with orthodontic treatment using Invisalign.

As the field of dentistry is constantly advancing and changing Dr. Forman spends a great deal of time going to seminars and continuing education courses to be able to provide the most up-to-date care for his patients. Dr. Forman is a member of several professional organizations including the Ontario Dental Association, the Canadian Dental Association, the Academy of General Dentistry, the AAID (American Academy of Implant Dentistry) and a member of the Spear Study Club of North Toronto. Dr. Forman’s philosophy is to work with his patients as a team so that they achieve the best care possible according to their desires, their needs and what is necessary to have the best health possible.


Lindsay - Office Manager / Treatment Coordinator


Lindsay is our Oral Health Facilitator/Team Leader. She is exceptional at what she does because of her positive attitude and enthusiasm for the dentistry provided by Forman Dental. With her 19 years’ experience in the dental field, she strives for excellence in customer service and office skills and continues to upgrade her knowledge of the latest innovations in dentistry through numerous dental education resources.

Lindsay is our go to person for everything dental! In her personal time she loves relaxing at home with her family or traveling to visit friends. Her favourite thing in the world is her Dog Dobby!


Marie - Practice Administrator


Marie has worked in the dental field for 7 years. She is a resident of Aurora and enjoys spending time with her friends and family at the cottage.

You will be received by Marie’s friendly smile and her eagerness to help. She is excited to be a part of the Forman Dental team and enjoys working within the Maple community. Marie will ensure that every visit is a positive one and you leave with a smile!


Pam - Oral Health Hygienist


Pamela’s love of Dental Hygiene began at a very young age. Her energy and enthusiasm has continued through the twenty-three years she has practiced in both general and specialty dentistry. She has a real ability to connect and understand the individual needs of each of her patients. Pamela’s commitment to oral health is unparalleled.

Outside of dentistry she loves the outdoors and spending time with her three children and Australian labradoodle Hugo.


Marsha - Oral Health Hygienist


Marsha's career in the dental profession in Canada started in January 2008 as a Registered Dental Hygienist. Marsha is also a foreign trained dentist from the Philippines. Marsha is inspired to help her patients have the best oral health possible and what inspires her most is seeing the improvements as she continues to work with her patients.

She enjoys family time with her husband and three-year-old daughter when she’s not in the office. As a family, they love being outdoors. She enjoys working out and photography.


Barb - Care Assistant / Clinical Co-ordinator


Barb is a graduate from the class of 2011 at Everest in the Chairside Dental Assisting program. Barb feels that working in the dental field is exhilarating and she loves getting to know her patients who she feels are an absolute delight.

Barb is of Polish decent and considers family very important. Away from the office her priority is being a mom to her 12 year old son and with spending time with the rest of her family. Barb encourages many new faces to come meet our dental family!


Charlene


Charlene graduated from the Oxford College Dental Hygiene Program in July 2014. We are excited to have her as the newest addition to the Forman Dental Team.

Charlene loves to help others with their oral hygiene and oral health. She is energetic and social, she loves to connect to all her patients. Charlene is passionate about dental hygiene and is always eager to help her patient do the best they can for the teeth and gums.

Charlene loves to bake and cook in her spare time. She enjoys going out to try out new restaurants. Food is a huge part of her life! Ask her about her world famous cupcakes at your next appointment.
